If a taxpayer is sharing a policy with another taxpayer or tax family you must complete Part 4 of the ACA Questionnaire.
Complete the ACA Questionnaire Part 1 and Part 2 (only use line 3 and enter totals exactly how they are).
Navigate to Part 4.
In A, enter the appropriate code for allocation situation and then enter the policy number.
B, enter the SSN of the taxpayer sharing the allocation (this will not be anyone on this tax return).
In C, enter the allocation start month.
In D, enter the allocation end month.
In E, F, and G enter the allocation percentage being claimed by THIS taxpayer. If the taxpayer’s cannot agree on a percentage, then it must be 50%.
